Eligibility Criteria

Inclusion Criteria

Patients presenting within 90 days of a transient ischemic attack or mild stroke in the MCA territory, who either have no history of cerebral ischemia, a history of ischemia exclusively in the region of the qualifying event, or a history of cerebral ischemia in any region with indeterminate etiology, who were angiographically diagnosed with 50-99% MCA stenosis within 7 days of the qualifying event, who have =< 50% stenosis of the ipsilateral extracranial or intracranial internal carotid artery, and who have a modified Rankin scale (MRS) score of =< 3, will be eligible for this trial.

Exclusion Criteria

Premenopausal females who are pregnant or not using contraceptives, and persons whose symptomatic artery is completely occluded, who have a history of cerebral hemorrhage, who have an absolute indication for or contraindication to chronic oral anticoagulant or aspirin therapy, who have any comorbid illness potentially associated with an increased risk of ischemic stroke, who have any potential source of aortic or cardiac emboli, who have any illness limiting life expectancy to < 1 year, or who, in the opinion of the investigators, will be difficult to follow up for any reason, will be excluded from this trial.
