European Carotid Surgery Trial

Status:

Purpose:
To assess the risks and benefits of carotid endarterectomy in patients with recently symptomatic carotid stenosis.
Interventions:
Endarterectomy, carotid Surgical therapy to remove atherosclerotic plaque in narrowed carotid arteries.

Design:
Multicenter, randomized, controlled trial of 3024 patients at 97 centers.
Inclusion Criteria
Some degree of carotid stenosis and at least one transient or mild symptomatic ischemic vascular event in the distribution of one or both carotid arteries within the previous six months.
Patient Involvement:
1811 were patients randomized to surgery, 1213 control (surgery avoided as long as possible).
Primary Outcome:
Major stroke or death over mean follow-up period of 6.1 years.
Results:
The risk of major stroke or death was not reduced significantly and did not vary substantially with the severity of stenosis within 2-3 years of randomization, but there was an 11.6% benefit (determined by Kaplan-Meier estimate) from surgery after 3 years in patients with stenosis >greater than 80% in diameter. Variations in risk with age and sex were found to modify the results.
